Hold Harmless Agreement: I agree to indemnify and hold harmless the Northern Hockey Development staff, Essentia Heritage staff, and Mars Lakeview Arena staff/employees from any and all claims, actions, suits, costs, expenses, damages, or liabilities, including attorney's fees, arising out of my participation in the ice hockey camp.

Assumption of Risk: I understand and accept the inherent risks associated with playing ice hockey, including both on-ice and off-ice activities such as practices, scrimmages, weight training, running, and related events. I am aware that these activities involve the risk of injury and illness.

Release of Liability: I release and discharge the Northern Hockey Development staff, Essentia Heritage staff, and Mars Lakeview Arena staff/employees from any and all liability for personal injury, property damage, or wrongful death arising from my participation in the ice hockey camp. This release includes, but is not limited to, injuries caused by the negligence or fault of the released parties.

Medical Treatment Authorization: In the event of injury or illness, I authorize the Northern Hockey Development staff, Essentia Heritage staff, and Mars Lakeview Arena staff/employees to seek and provide medical treatment as deemed necessary. I understand that I am responsible for any medical expenses incurred on my behalf.

Photography and Likeness Release: I grant permission for the use of my photograph or likeness in promotional materials related to the ice hockey camp, without compensation.
